Accident Investigation Services
OSHA requires an investigative report of an accident to include appropriate documentation on date, time, location, description of operations, description of accident, photographs, interviews of employees and witnesses, measurements, and other pertinent information.
Community Air Monitoring
PHASE Associates construction site safety professionals offer community air monitoring services to identify and alert our clients of airborne contaminants that are suppressed and confined to the property site under construction or being remediated.
We also develop and implement Community Air Monitoring Plans (CAMPs) following applicable federal, state and municipal regulations. We would develop an air monitoring strategic plan, provide the applicable air monitoring instruments, datalog and field log the data/ documentation, report and alert our building and remediation client Project Managers of air quality exceedances.
Confined Space Entry
Establishing operating procedures, identification of hazards, controls, testing, safety requirements, emergency response plan, emergency services response for permit confined space rescue, and ventilation requirements.
Confined Space Entry Rescue and Retrieval
Customize permit-required Confined Space Entry Rescue and Retrieval Team procedures and action plans. Our safety professional and Confined Space rescuer/First Aider specialist team up to provide a comprehensive documents.
Construction Outreach Training Program
A PHASE Associates OSHA Authorized Construction Safety instructor will conduct this training. 10-hour | provide construction personnel information about their rights, employer responsibilities, and how to file a complaint as well as how to identify, abate, avoid and prevent job related hazards on a construction site. 30-hour | provides a variety of training to workers with some supervisory and safety responsibility.
Ergonomic Reviews
Identify hazardous ergonomic work practices or conditions at your work site. We will evaluate to develop practical solutions, and create simple and cost effective recommendations. We can assist in the establishment of an ergonomics program.

Health and Safety Plan (HASP)
Health and Safety plans are also commonly referred to as Safety Programs and Site Specific plans which is a major part of OSHA compliancy. PHASE Associates is proficient in providing your company in developing a new health and safety plan or updating an existing one, all customized to meet your standards, policies and procedures. The program components are based on the constructions standards from OSHA while complying with all state and federal regulatory requirements.

Job Site Safety Analysis/Inspections
Job safety analysis (JSA) is a tool that can identify and offer controls associates with specific job tasks, processes or procedures. It is a detailed examination that looks at each job step to identify the health and safety hazards and recommend controls for each. OSHA recommends JSA as a very effective means to helping reduce incidents, accidents and injuries at a job site.
We offer the following services for JSA:
- Observe the workplace and job processes
- Break the job into sequential steps
- Identify physical and chemical hazards
- Recommend safe job procedures for each step
- Provide training and instruction on the JSAs
Lockout/Tagout
OSHA Recordkeeping Management
Our construction site safety officers review OSHA’s 300 logs of injuries and illnesses, first aid logs, workers’ compensation records, and any other accident/incident internal logs to ensure regulatorily and workers’ compensation compliance. Training sessions on proper OSHA recordkeeping practices are also available to maintain compliance.
OSHA Voluntary Protection Program (VPP)
Assist firms with their OSHA VPP applications and prepare them for the required on-site audit.
Personal Protective Equipment (PPE)
Perform PPE hazard assessments for all protective equipment devices and prepare the written assessment. Evaluate of the types of personal protective equipment to purchase and use according to the hazards identified. Respirator fit testing is also available.
Respirable Crystalline Silica
PHASE Associates will provide the necessary support to protect personnel against respirable crystalline silica inhalation hazards. We have experienced industrial hygienists that can develop and implement engineering controls and work practices; provide training and conduct air measurement sampling and silica dust surveys.
